I need a quote for tooling and production of a plastic part, which type of information do I need to provide?

The following are the basic documents and information that we need to quote a new tool and production of injection molded parts: 

• 2D drawings of the part with dimensional tolerances and parts requirements
• 3D models of the part in format compatible with Solidworks 2009 if available
• Plastic type and grade that will be used for part production
• Description of the part application
• Agency requirements (UL, FDA, etc..)
• Expected product life
• Expected order quantities
• Packaging requirements
